Eligibility: An applicant must meet HUD’s eligibility criteria including: meeting income limits; family size. All public units are 3 bedroom homes; furnishing social security numbers for everyone in the household over 6 years of age; meeting the definition of a family, may not owe a debt to any housing authority; a preference will be given for working families, elderly and disabled as well as other preferences.

Procedure:
Families may apply by completing a written pre-application card which places them on the waiting list. When their name reaches the top of the list, the applicant will be sent a letter inviting them to a Pre-Occupancy briefing and classes. Once eligibility is confirmed and classes have been attended, the family is screened for additional suitability. When the family is determined to be eligible on all accounts, they are placed on a short “waiting list for a vacant unit” list, as soon as a unit becomes available for occupancy the family is asked to sign a lease, pay a deposit and move in. You will work closely with the PH Coordinator to ensure your move in is a smooth process.

Information:
Families need to be aware tha failure to abide by the terms of the lease may be grounds for eviction. Families are required to keep income and composition information up to date and must recertify annually per HUD regulations.

Eligibility: Eligible clients must verify homelessness and low income status. Single adults only. Unable to serve sex / violent offenders. Clients must be willing to work with case manager. A metro voucher is also required.

Eligible clients complete application from and receive social service interview. Once eligibility is established client is placed on waiting list and admitted once an apartment is available.

Information:
Fairhaven Place is a permanent housing program for homeless adults. Each unit is an efficiency apartment. Residents must accept case management services designed to assist them in moving towards self-sufficiency.

Complete application and complete intake interview required. Must meet low income requirements and be willing to work towards identified goals.

Programs is designed to move homeless families to self sufficiency. Provides education and referrals to community resources. Limited financial, transportation, and food assistance available. We also provide monthly parent support groups.

Procedure:
May contact the office to complete a written application by calling 740.653.6618. Applicants will be placed on a FSS waiting list and once pulled from the waiting list, will attend FSS individual case management. At the meeting, applicants may elect to sign a 5 year contract of participation.

Information:
Participants will receive case management services to develop family case plans, identifying specific goals that will enable families to achieve family self-sufficiency and eventually be free of all public assistance. The participating family will meet with the FSS coordinator on a quarterly basis and review the case plans and make appropriate adjustments. As earned income increases, an escrow account is established that will accumulate on a monthly basis as long as income is stable and or continues to increase. This escrow account can be used during the 5 year contract period to assist in meeting goals such as education and transportation. Once the family has successfully graduated from the FSS program the balance of the escrow accoount is paid to the family.
