Arranging Your Home Care – Our services may be requested directly by you or a family member. We coordinate the services with your physician. You don’t need to be discharged from a hospital to qualify for our services.
First visit – Usually the nurse visits the patient within 24 hours of the referral, often the same day if returning home from a hospital or facility stay. Therapists almost always begin treatments within two days. Services We Offer – Professional Nursing Care, Physical Therapy, Occupational Therapy, Speech Therapy, Social Services, and Personal Care Services. |

AlternaCare is a Medicare certified agency offering an alternative to extended hospitalization or stay in a nursing home. Our nurses are on-call 24 hours a day. We provide in-home services throughout Fairfield and part of surrounding counties for individuals who need assistance to live indpendently. Among those we serve are people coping with a chronic disease or rehabilitation from illness, injury, or surgery; with recent medication/treatment changes, at risk for complications; and recently discharged from a hospital or extended care facility. |
